This product will be EMAILED to you, to be printed by you at home or by a professional.

Consultation forms are a very important part of your treatment. This allows you to take the necessary information from your client, not only to ensure you can contact them if needed, but to check they are safe to have the patch test and treatment completed.

These forms are double sided and include a record card to be printed on the back.

These are designed to be printed by yourself at home, or printed by a professional company. You will NOT received a hard copy/paper copy of this product. You will receive your designed forms via email and will need to print them/get them printed yourself.

Once your order has been placed, please email with a high resolution JPEG image of your logo. If you do not have a logo, we can simply use write your business name on the form.

If you wish to have any additional information added to the form, such as your website, please let us know in the email.

Once the product has been designed, and you have agreed you are happy, any further amendments to the document will incur a £5 fee.

Please allow up to 5 working days for your design to be returned to you.
